CASE MANAGEMENT

The Aquarius Project's target population is at-risk and high-risk youth. Many of these youth will require some level of assistance and access to supportive services in order to move forward in achievement of their academic and personal success. Aquarius staff will screen and assess each youth for functioning across major life domains, with the use of the Santa Clara County Functional Assessment Scale (FAS) and, if indicated, the ASAM (American Society for Addictive Medicine) six-dimension scale. These tools, along with case records, also serve as performance measures in project evaluation as they serve as pre and post-tests to determine the relative change in levels of functioning across multiple life areas and with regard to use of substances.

Additionally, Aquarius staff will connect with parents through home visit to include them in any treatment planning and to orientate them to the project.

Finally, Aquarius staff will make referrals to community service providers and provide linkage, transportation, and monitoring to ensure the appropriate communication and support of engagement and progress in treatment.
